Work Permit Info

Work Permit Requirements:  All students under age 18 require a work permit to work.  Once you turn 18, you do not need to apply for a work permit.  Work permits are valid for one school year (September 1 through August 31).  You must maintain a 2.0 GPA and regularly attend school to qualify for a work permit. Your work permit can be revoked at any time if your grades or attendance drops.
 
Step 1: Fill out a Work Permit Form
You need to fill out one of these two work permit forms.  For your convenience, we provide two different formats of fillable documents: one is a fillable pdf and the other is a fillable word document.  The form must be signed by you, a parent, and your employer before submitting it.

Step 2: Submit Your Work Permit Form
Once you have filled out the form (which must include your social security number),bring your completed work permit application form to Ms. Seliger  rseliger@seq.org.  Ms. Seliger will process your work permit and then it can be picked up at A-15.  Drop off and pick up can be on Monday, Tuesday, Fridays during lunch hours.

Step 3: Turn in Your Work Permit Form to Your Employer 
Once you receive your work permit, you need to sign and date the form.  You are not allowed to start working until your employer has received your signed work permit form.  Don't forget you need to maintain a 2.0 GPA to retain your work permit.
